Create a simple Microsoft Word DOcument Template - Upwork

We need a Microsoft Word Document designed (That can be easily edited)

1.  page 1 - same as 'Sample personal details’ attachment
2.  page 2 - same as 'Terms and Conditions for Admission’ attachment and should be on the reverse side and/or an option of printing it on a second page.  

The format for the Terms and Conditions should be the same style as the Terms and Conditions on the ’NC Admission T&C’s.PDF’ which is attached ie. headings on the left and provision for signatures at the bottom of the page.

Please note that I require the form in a format that can be edited from time to time to include new T&C’s or new Personal details fields etc.  We will also be making a few more inclusions in the T&C’s in the coming days so this will not be the final document.
